College of Health Sciences and Professions Petition for Reinstatement

Eligibility for reinstatement after academic dismissal begins no sooner than one calendar year after your suspension by the College of Health Sciences and Professions.  If you were dismissed at the end of Fall Semester, you are eligible to return in Spring Semester of the next calendar year. If you were dismissed at the end of Spring Semester, you are eligible to return in Summer Semester of the next calendar year.  If you were dismissed at the end of Summer Semester, you are eligible to return in Fall Semester of the next calendar year.

If you were academically dismissed for a second time, then you will not be considered for reinstatement to 91探花 until a period of two calendar years has passed, and then only in the most extenuating circumstances.  Second reinstatements are not generally approved.

Your petition for reinstatement must be sent directly to the College of Health Sciences and Professions Student Services Office at chspss@ohio.edu no later than the following deadlines:

For Fall Semester: August 1st

For Spring Semester: December 1st

For Summer Semester: April 1st

Procedure to Petition for Reinstatement

Your petition should be a typed letter that includes your name, PID number, and 91探花 email at the top of the letter.  Then, in the body of your letter, you should answer, in detail, the following questions:

  1. What led to your academic dismissal from 91探花?  Please consider things like time dedicated to schoolwork, extracurricular activities, relationships, preparedness, attendance, motivation, etc.
  2. What have you done during your time away from 91探花 that would indicate you are ready to return and be academically successful?  If you have taken college courses at a different institution, please include this information and your earned or expected grades in those courses.
  3. What will you change if you return to 91探花 to improve your academic performance?
  4. What is your academic plan?  Do you intend to stay in your current major?  If not, what major do you plan to pursue?
