All ISSS forms must be signed by the academic signatory authority in the college or department. Each form lays out the proper signatory. If you have questions, please contact isss@ohio.edu.

F-1 Students

  • Recommendation for F-1 Program Extension

    Fill out this form if you are a student who wishes to extend your program time.

  • Recommendation for a Reduced Course Load for Medical Reasons

    This form is used as a recommendation for a reduced course load for medical reasons.

  • Reduced Course Load Recommendation Form

    This form must be completed by a representative from the student services office in the student's academic college to help the ISSS team determine if the student may be approved for a reduced course load.

  • CPT Supplemental Form

    This form is required with a CPT application if the internship is needed for the successful completion of a thesis or dissertation.

  • OPT Recommendation Form

    This form is for F-1 students seeking permission to engage in Optional Practical Training (OPT), an employment authorization provided by the United States Citizenship and Immigration Services (USCIS).

  • Re-Enrollment Instructions and Request Form

    An F-1 student must complete specific steps to return to 91探花 and complete the request for re-enrollment form.

J-1 Students

  • Recommendation for J-1 Program Extension

    Fill out this form if you wish to extend your J-1 program.

  • Reduced Course Load Recommendation Form

    This form is used as a recommendation for a reduced course load for J-1 students.

  • Academic Training Recommendation Form

    This form is used for students who wish to apply for Academic Training work authorization.

    Please ask your department to complete this letter template on department letterhead. If you will be working a different number of hours in different semesters, then they should submit a separate letter for each semester.
